A: I voted twice natural and a little unnatural. The sentence is grammatically correct. However whether it's natural or not is based on the situation.

It's a completely natural to ask someone if they need help in this manor if you are working and talking to a client. For example the clerk at a hotel may ask "Do you require assistance with your bags?" It would also sound natural if the question was posed by a police office, fireman, or paramedic to someone who appeared to be in need of help.

However, if you're at the supermarket and see a person struggling with their bags, this would sound too formal. You would general hear "Do you need help?" or "Can I help you with that?"
