"Again" is used when the same thing happened more than once, or if someone does the same thing more than once. For example, "I drove to that restaurant again".
"Over again" is never really used, people usually say "all over again", and that is used when a process happens again or when it's repeated. For example, "doing you homework" can be a process. Let's say you lost your homework, so you'll have to do it again. So people will say "I lost my homework, so I have to do it all over again", which means that the person will have to repeat the process of "doing their homework" again.
"Over and over again" is used when the same process happens a lot of times or if someone repeats the same process many times. For example, "throwing a ball" can be a process, so when someone throws a ball repeatedly, you can say: "that person is throwing the ball over and over again".

You can think the difference in terms of frequency and repetition. I can give you a couple examples:
1. Did you fail classes again today?
2. Of course you do. You've been doing the same thing in the last month over and over again.
3. Now we need to do all your homework all over again.

For all over again? You say "all over again" when you have to repeat something for usually the second time (though it can be third, fourth and so on) because something bad happened the first time. For example, if you are running on grass with a coin and you drop it, but you don't know where you dropped it, and you only realized that you dropped only after you stopped running, then you have to "run that path all over again" so that you can find the coin.
