I don’t think “I’m by you” is said as often as the other two, but they all mean the same thing... the difference between saying “disappointed in you” and “disappointed with you” is like the difference between saying “someone” and “somebody.” I will say that I use “disappointed in you” more often that the other two (it’s almost like it’s the generic form... like maybe more common)
-I’m disappointed in you/myself/him/her/them for being so careless.
-I’m disappointed by Sonic’s new menu (“disappointed by” is typically used with objects/nouns, not people)
-He disappointed with himself for not having run faster/He’s disappointed with the Town Counsel’s latest bill (used with both people and things just like “disappointed in”)
-I’m disappointed in his lack of self-responsibility.
-I’m disappointed by their performance tonight, usually they’re better than that.
-I’m disappointed with you. You need to go back and apologize to Mr. Duncan for what you did even if it was just an accident.

All are wrong, each verb has its matched prepositions. In this case, it should be:
I'm disappointed at you.
