Never had anyone ask is in the past tense, and in response to a question most of the time. No one ever asked is also in the past tense, but it can be used as an answer to a question and also as a statement on its own, whereas the first sentence is more of a response to a question only.

Never had anyone ask would mean no one bothered(tried) to ask in the first place.
Ex:"I never had anyone ask how I felt."
No one asked would mean what it says – no one asked.
Instead of saying "No one never asked", just say " No one asked."
Ex:"No one asked you to do anything."
They basically mean the same thing or at least very close to be honest.
