From my experience, although it is more uncommon to write the same sentence structure of B, I think it is still helpful to learn it.
The difference is very simple. "A ( noun ) is when explaining something in general or when you do not want to give context.
The ( noun ) is when explaining something that is already previously said and when the context is already given.
For example, when I say " I am swimming in a river. " You do not know where is the river because this is a general sentence.
However, when I say " I am going to Okinawa, and I want to go swimming in the river. "
In this sentence, "the river" is referring to the river near Okinawa by context.

@pomf1129 “the” = definite article; the river is a determinate one, supposedly known, or identified by the context for instance.
“a” = indefinite article; a river, could be any river.

One more thing, let's say all of a sudden you want to go fishing, but you don't know a proper river to fish. As you don't know a specific location, can you say "I want to go fishing in a river."? Is it grammatically legal? I want to know situations where B sentences come into play.
