gradually = little by little, slowly
(This doesn't specify time frame. Gradual change can happen over seconds, all the way to centuries. It depends on the context.)
e.g. I'm gradually getting better
e.g. Her health was gradually declining
day by day = every day something is slowly happening
(The change is noticed daily)
e.g. I felt myself becoming stronger day by day
e.g. Her health was declining day by day
day after day = every successive day
(suggests that it is a long time period)
e.g. It rained day after day (= it rained many days in a row)
e.g. My job kept me doing the exact same thing day after say
