1) Sounds like I said or did something that made them angry or sad.
Ex:
A) I think books are annoying. 😑
B) But I love reading books. 🙁
A) Sorry if I offended you. I didn't know you liked books. 😓

"uncomfortable" in this context means "I don't like being near you right now."
Unless the person is mean, this is usually more accidental than 1.
Ex:
A) I love eating burgers 😁
B) I'm a vegetarian. This is making me uncomfortable. 😒
A) Sorry.😓
Here's an example of a mean person.
A) What's your name? 😜 (they ask this 100 times)
B) This uncomfortable. I'm leaving. 😖
"offended" is stronger than "uncomfortable."
In 1, B felt that A was insulting him.
In 2, B didn't like talking about meat. But it wasn't that bad.
You can be uncomfortable without being offended.
Even native speakers (and me!) get a little confused about these words haha.

"offended you" usually means "made you angry" or "made you upset" (怒る、腹を立てる)
"made you uncomfortable" could mean "made you angry" or "made you sad" or "made you scared" or "made you uneasy" (不安、居心地が悪い)
If someone told me I looked ugly that would offend me
But if someone stared at me on the train for a long time that would make me uncomfortable
"I'm sorry if I offended you" would mean you are apologizing because you did or said something to make the person angry or upset
"I'm sorry if I made you feel uncomfortable" would mean you are apologizing because you said or did something to make the person feel in some way no longer safe and comfortable
