Well, I guess “My Dear” is more sincere than just “Dear”. For example , “I love you, dear!” and “I love you, my dear!” The second one is more sincere and genuine compared to the first. “My dear" is usually for people who are closer. It is a term of endearment - someone who is dear to you.
