The difference is Grammar. To say “one of the guys” is more formal and grammatically correct and is used more often however, to say “one of them guys” would be informal and may be used in a less professional environment.

It'd be really context-dependent.
Without any other context, I'd assume "one of the guys" is "one of the friends in our social group" and "one of them guys" is "one of the people in the group that we're talking about".
