They practically mean the same thing. But when using also, you would use it as an add on to a sentence. While, as well is often times used for listing.
Also sentence:
"I also forgot to mention I won't be here tomorrow."
As well sentence:
"She went to the store to buy eggs as well as milk"

They are nearly interchangeable but you have to change the order of the words sometimes. To piggyback on the previous examples:
"She went to the store to buy eggs and milk also."
