why are you holding a cat - なぜ猫を持っていますか。（とくに驚いてないし、不思議に思ってない)
Why, exactly, are you holding a cat? なら「一体なんで猫を持ってるわけ？」って感じ。"exactly" を加えるともっと「意外」とか「おかしい」とか「怒り」の気持ちも入ってる。ようするに、強調を表してる。
