American: center, realize, organize, theater
British: centre, realise, organise, theatre
Also some words are completely different:
Sidewalk (US) - pavement (UK)
Chips 🇺🇸 - crisps 🇬🇧
Fries 🇺🇸- chips 🇬🇧
Candy 🇺🇸- sweets 🇬🇧
Eggplant 🇺🇸- aubergine 🇬🇧
There are hundreds more but that's all I can think of now. I think both countries are so familiar with each other's language, vocabulary and accents (through TV and movies etc) that we can understand each other fine. (Except Americans have trouble with some British speakers with really strong local accents, like from the countryside, and vice versa)

The pronunciation is can be different and some of the vocabulary is different. There are also regional accents within both countries but for the most part, we understand each other with not too much difficulty.

American: color, harbor, favorite
British: colour, harbour, favourite
Not a big difference but some British words have "ou" compared to "o" in America.

I mean, in a conversation for example, when an American talks to a British, will the British understand everything the American says ?

In the US we like to watch British TV shows and read British novels and we understand them just fine.
