'An' is used for words that begin with a vowel, for example-
"An apple"
"An elephant"
"An igloo"
"An orange"
'An' is also used for any consonants that begin with a vowel sound, for example 'X' as it beings with an 'eh' sound-
"An x-ray"
"a" is used for all consonants that don't begin with a vowel sound,
"A dog"
"A cat"
"A school"
However, "a" and "an" can both be used for the vowel 'U'
"An umbrella"
"A unicycle"
This is because it makes two different sounds, "uh" and "yoo"
The first sounds like a vowel therefore uses 'an' but the second sounds like a consonant therefore uses 'a'
In summary,
'An' is used for a word beginning with a vowel
'A' is used for a word beginning with a consonant
The exceptions being 'X' and 'U'

13 and 16 do not require either 'a' or 'an'
"She is filled with anxiety"
"Mr. Berkley has come here four times today"
This is because neither are an object, there is no need for 'a' or 'an'
Other than that they are all correct! :)

@emilileigh Slight correction. "A" is used for any word NOT starting with a vowel sound. So as well as a unicycle, we also say a European....
Of course we also say an MP3 and an SMS, because these start with vowel sounds.
