l think that you can use did to emphasize a question when, 'What, plus a verb' is the subject of the sentence. For example....What caused the accident ? If it is not clear what another person is telling you, you could say: What exactly did cause the accident ? However, I stand to be corrected eventually. What happened to you this morning? What happened to you yesterday, last week, etc. What has happened to you, is something that you can see immediately, and you use this tense ( Present perfect ) for something that started in the past and continues in the present....e.g. What has happened to you since you came back from Italy ? You just haven't been the same since then.
