have happened - it means that something happened and is still happening today. For example "Mercedes has been making cars for a long time now" They are still making cars so you need a past tense with "have" here.
Had happened- it means that something had happened before another thing happened. For example : He had eaten a slice of pizzs before he went to the gym.
So
Have + (been) + 3rd verbform
-> still happening
had + (been) + 3rd verbform
-> Explains an action that had happened before another one.

“Have happened” - These infuriating situations have happened to him before, so he knew how to handle them easily.

Have - Past continous
Had - Past complete.
The toys falling on the ground have happened before.
(Have happened suggest it's an on going issue)
The toys falling on the ground had happened before. (Had happened suggest its no longer an issue)
