When someone says "I could've helped you then" It is past tense, meaning it is too late to help.
When you say I could help you then it is present tense meaning you are able to help them still.

Let's say you are having trouble with homework. I would say "I could help you."
But if you had trouble with your homework last night, school has already began so I may say "I could've helped you."
Does that make more sense?
