Perhaps a better way to ask would be: "What makes a person "highly skilled" at speaking English?"
An answer might be: "A 'highly skilled' speaker of English is able to use the best words for each situation, not necessarily the most difficult words."

Do you mean what kind of English does someone need to speak for others to consider them 'highly skilled'?
It depends. Using difficult/complex words can definitely make you sound 'skilled', but they can also make you sound pretentious or like you're complicating things unnecessarily if they're used too much (or in the wrong situation).
For example:
A: "That was a great movie."
B: "Indubitably."
In its simplest form, "indubitably*" means "yes". Indubitably is a 'difficult word', but B doesn't sound 'highly skilled' - using an overly complicated word when there's a simpler one with exactly the same meaning makes him sound a bit odd.
Instead of focusing on difficult words, I think the people who appear highly skilled at English are the ones who understand the meaning and usage of basic everyday words and can use them in the right situations.
*"Indubitably" means "undoubtedly" (without doubt), so when it's used as an agreement ("that movie was great" "indubitably"), it has the same meaning as "yes".
