She would like to know how the aroma necklace works.

I wonder if she would like to know how to use the pendant, like how to fill aroma oil in the pendant, or if she would like to know the effect from it.

Both are possible.
Really, "how __ works" is more like your second explanation, and "how to use" would be
"She would like to know how to use the aroma necklace" なんだけど、both meanings are possible.

"how ___ works" would normally be answered by something like:
"The oil is stored in a porous bladder inside the pendant, and the oil is slowly released through pores into the surrounding air, giving off the desired scent" などなど。
