I think: #1 it is a statement. Someone has already invited you to fill out the application. It's part of the past. #2 is more present. Right at this moment you have been invited to fill out the application.

1# You use (.) when you are NOT asking a question but is stating what you know 🙂. 2# You use this (?) to ask questions or when you feel confuse😕. You won't see " You're welcome to fill out an application with a (?)" unless the person is not sure that you should be filling out the application at the first place (That shows that the person is unprofessional because it means that the person doesnt know what they are doing)
